Register a device on the Physics network

In order to register your computer in our DHCP Table (and thereby gain Internet/Network Access), you will need to complete this form.

Name*
First Last
Gatorlink Username
Enter or verify your gatorlink username.
Office Number*
If you are in the basement level include the "B" in the office number. Example, B134, B102, etc.
Computer Name*
We use this to separate personally owned equipment from department owned equipment.
Computer Manufacturer and Model*
We use this and the computer name to keep personally owned computers from getting counted in the inventory of department owned computers.
Operating System*
Operating System Version*
MAC Address*
When typing in letters, please use lowercase.
Other information
Let us know if you have a special situation that is not covered by this form. Examples - you need multiple IP addresses, your equipment does not ask for an IP address via DHCP.
